The Geriatric Long Term Care (LTC) Outreach Team is for residents of long-term care facilities who have a psychiatric concern that is either treatment resistant or complex in nature presenting with one or more of the following: cognitive changes, behavioural changes, mood changes, anxiety or psychotic symptoms.

For ages 65+
Inclusion Criteria
- Residents of long-term care facility
- Age of 65 or older
- Have a psychiatric concern that is either treatment resistant or complex in nature
- Presenting with one or more of the following: cognitive changes, behavioural changes, mood changes, anxiety or psychotic symptoms
Service Access
Referrals are accepted from long term care facilities.
